Diabetes affects 16 million Americans and accounts for nearly 13 percent of all health care costs. This Picker Program Grant will support collaborative efforts among health plans in three sites to improve the quality of care for diabetic patients. If the project is successful in improving the health of those with diabetes, it could point the way toward similar efforts in which health plans work together to raise the quality of care for others. The American Association of Health Plans will provide cofunding in-kind.
