In April 2008, the Fund's Board approved a five-year initiative to transform 50 safety-net clinics into patient-centered medical homes, which several studies indicate can improve quality, reduce costs, and narrow disparities in patient care. To assess the effectiveness of this ambitious initiative, a team based at the University of Chicago will evaluate whether the participating clinics in fact become medical homes, how medical homes affect quality and efficiency, and what factors are associated with a clinic's successful implementation of this care model. The project team will draw from organizational and patient survey data, interviews with clinic staff, a review of clinical data, and patient claims data (to determine the initiative's financial impact). If the first phase proceeds satisfactorily, additional phases of funding will be requested.
