Assessing and Reducing Medication Errors in Hospitals


Errors made in prescribing or administering medications cause approximately 7,000 deaths annually in the United States. To help hospitals adopt safer medication practices, a self-assessment tool jointly developed by the Health Research and Educational Trust and the Institute for Safe Medication Practices was recently distributed to all hospitals and health systems in the country. This Picker Program Grant will follow up with 1,000 hospitals to determine if they have used this tool, to encourage hospitals that have not conducted a self-assessment to do so, and to collect information on the state of medication safety practices nationwide. Cofunding will be provided in-kind by the American Hospital Association.
