Exemplars of Local Health Care Delivery Reform: Where Policy Meets Practice

This project will generate knowledge about how local health care organizations and stakeholders are continuing to transform health care delivery to improve quality, efficiency, and health. In previously supporting the project team at the Institute for Healthcare Improvement, The Commonwealth Fund has assembled a highly regarded set of resources on innovation in health care delivery. Over the next year, the team will focus on how health systems are making the changes necessary to deliver population-based care and how they are targeting high-risk populations to ensure maximum impact on health and costs. The team will also revisit three to four examples of high-performing delivery systems first profiled in 2008 to see how they have evolved and adapted to the new health care environment, particularly in light of recent reforms and new accountable care programs. The case studies and synthesis briefs prepared by the team will inform policymakers and delivery system leaders about the crucial interplay between policy and practice.
