Spreading Use of a Health Care "Hotspotting" Tool to Improve Quality and Reduce Costs

The Camden Coalition of Healthcare Providers, in Camden, N.J., has worked with local providers to map where the biggest users of emergency department and inpatient hospital care reside in this high-risk community—a process known as "hotspotting"—and implement interventions to improve care coordination and quality and reduce costs. Given growing demand nationally for the data analysis and mapping techniques used by the coalition, this project will support the development, dissemination, and testing of a health care hotspotting analytic toolkit that can be used by communities motivated both to improve quality of care for their high-risk, high-need residents and to reduce total health care costs.
