Strengthening Primary Care Through Multipayer Medical Home Initiatives


The effectiveness of medical homes, and their sustainability over the long term, will depend on broad participation by a range of stakeholders, including public and private payers working together. With the participation of multiple payers, primary care practices are better able to make investments in staff and infrastructure needed to improve performance, since practices receive enhanced medical home reimbursement rates for a greater proportion of their patient care. Since 2007, The Commonwealth Fund has supported the National Academy for State Health Policy in tracking and assisting state Medicaid programs with medical home implementation. This project seeks to accelerate multipayer initiatives in four states by helping public and private payers reach agreement on key issues, such as medical home qualification standards, payment models, patient attribution methods, and evaluation metrics.
