Independence at Home Learning Collaborative

The federal three-year Independence at Home Demonstration program is testing whether primary care services delivered in the home can improve access to care and lower costs for treating high-risk Medicare and Medicaid beneficiaries whose poor health and frailty make it difficult or impossible for them to obtain adequate primary care in doctors’ offices or clinics. This grant will support technical assistance for a collaborative of 15 primary care practices and programs chosen by the Centers for Medicare and Medicaid Services (CMS) to participate in the demonstration. The project team also will facilitate feedback between the collaborative’s members and CMS officials. Cofunding is being sought from the Retirement Research Foundation.
