Safety Net Medical Home Initiative: Dissemination and Behavioral Health Integration

In June 2013, The Commonwealth Fund’s Safety Net Medical Home Initiative will officially conclude. For the past five years, it has assisted 65 community health centers in becoming patient-centered medical homes for their primarily low-income patients. Recent results from the sites show improvement on all important aspects of medical home transformation. This grant will help fill two information gaps that have come to light: the lack of evidence in the peer-reviewed literature to guide primary care practices, especially within the safety net, in the medical home process, and the need for a resource to help practices integrate behavioral health services with primary care. The journal articles prepared by the Qualis Health team also will be a vehicle for disseminating the resources and lessons generated over the course of the initiative and evaluation.
