Implementing New Systems for Integrating Care Provided to Medicare/Medicaid Dual Eligibles, Phase 3

To improve quality of care and control costs for people dually eligible for Medicare and Medicaid, the federal government provides opportunities for states to test new ways of delivering and financing care for this population. Commonwealth Fund support for two prior phases of this project has enabled the Center for Health Care Strategies to work with states in designing and implementing care plans for the dual-eligible population as part of the federal Financial Alignment Initiative. This third phase will continue to support states that will be in the midst of implementing their plans in achieving integrated care for dual eligibles in 2015 by providing opportunities for them to share their experiences on a peer-to-peer basis, interact informally with relevant federal officials, and provide support for states in focusing on issues of common interest.
