Change Package for Delivery Systems to Adopt Programs for High-Need, High-Cost Patients


Health care organizations recognize that a small percentage of the population accounts for a large proportion of health care costs. While there is new evidence that certain programs can improve outcomes and lower costs for high-need, high-cost patients, most providers and health systems lack the knowledge and skill required to implement such programs. Through the learning collaboratives it has conducted over the past two years for 60 health care organizations, the Institute for Healthcare Improvement (IHI) has developed the know-how required for successful implementation of interventions targeting clinically complex patients. What’s now needed is a practical resource that will help health systems take advantage of current best practices in the field.
