Surveying High-Need, High-Cost Patients About How They Experience Health System Reform


Informed by earlier Fund-supported focus group research, the new Commonwealth Fund Delivery System Reform Survey will collect data on a range of topics, such as: communication between care providers and patients; the doctor–patient relationship; integration and coordination of care among providers, including social service agencies; and patients’ difficulties in navigating the health system. The survey research firm SSRS will collect data through its Omnibus survey, which will yield a nationally representative sample of 3,000 adults, including an oversample of 1,800 people with multiple chronic conditions or functional limitations affecting daily living. Results will be reported in late 2016, in time for the transition of the new presidential administration.
