Authorization for a “Playbook” for Improving Care for High‐Need, High‐Cost Patients

The Commonwealth Fund, John A. Hartford Foundation, Peterson Center on Healthcare, Robert Wood Johnson Foundation, and SCAN Foundation all share a vision: to transform health care delivery for America’s sickest, frailest, and costliest patients. These foundations have formed a collaborative to coordinate resources and funding for accelerating progress toward this goal. As an initial step, the Fund has partnered with the Institute for Healthcare Improvement (IHI) to create a dynamic web resource for health system leaders engaged in alternative or value‐based health care payment models. Planned for launch in December 2016, this resource, or “playbook,” will demonstrate the challenges facing adults with complex health and social needs and provide direction for meeting those needs. This authorization will support further development and maintenance of the five‐foundation playbook and related activities. The authorization provides flexibility for the Health Care Delivery System Reform team to develop a concrete proposal with the IHI team and our foundation partners to identify new content, enhance site functionality, and collect user feedback. The requested funding will additionally support implementation of strategies to maximize the playbook’s outreach and effectiveness, as well as potential participation in a key forum such as the Aspen Institute’s Spotlight Health conference. Investing in the dissemination of playbook “1.0” and building a stronger, more comprehensive version 2.0 will help achieve the collaboration’s goal for this resource: to motivate and support accountable care organizations and Medicare Advantage plans, health system leaders, payers, policymakers, and providers in improving outcomes and lowering costs for high‐need, high‐cost populations.
