As use of home-based medical services, including both primary and palliative care, rapidly increases, so does the need for nationally recognized quality measures that are appropriate for a frail patient population. In the first two project phases, researchers identified promising measures and measurement gaps and developed a quality measurement framework for home-based primary and palliative care. In phase 3, the team will prioritize and refine its current set of indicators and test their reliability, feasibility, and validity before applying for endorsement by national measurement organizations. This work is essential for ensuring that quality measures for home-based care are incorporated into practice and linked to provider payment.
