Improving Care for Those Who Need It Most


Faustino Gazan, 47 years old, has diabetes, hypertension, kidney disease, and a genetic condition called Prader-Willi syndrome. He is what many in health care refer to as a “high-need, high-cost” patient—part of a growing population that experts say health systems should be targeting—both because they need the most help and because they have the biggest impact on national health spending.
