Webinar: How Are Federally Qualified Health Centers Serving as Medical Homes?

The patient-centered medical home is a new model for improving primary care delivery. But studies show patients in safety-net settings are less likely to report that their health care providers have the characteristics of a medical home. Listen to a recording and download the presentations from a December 4th webinar that looked at how one segment of the safety net is working to promote medical home practices in primary care. "How Are Federally Qualified Health Centers Serving as Medical Homes?" was hosted by The Commonwealth Fund's Program on Health Disparities. Kaytura L. Felix, M.D., chief medical officer of the Bureau of Primary Health Care at the Health Resources and Services Administration, defined the patient-centered medical home and addressed what federal regulations and resources support medical home practice in health centers. St. Anthony Amofah, M.D., M.B.A., chief medical officer of Community Health of South Florida, Inc., discussed implementation of the medical home in clinical settings and the barriers health centers encounter during this process. David M. Stevens, M.D., F.A.A.F.M., director of the Quality Center of the National Association of Community Health Centers, reviewed the benefits of the medical home model for community health centers and discussed where the medical home fits into broader health reform.
