A Practical Guide for Healthy Development

A Practical Guide for Healthy Development is a set of materials and tools designed and tested in the Healthy Development Learning Collaborative, a 12-month quality improvement initiative. The initiative was designed to help primary care practices in Vermont and North Carolina engage families in a partnership to promote positive developmental outcomes for the families' children through the development of improved office systems.

The purpose of the guide is to provide primary care offices with the tools and resources to accomplish the following:

  • identify more efficient practice-based systems to organize preventive and developmental care;
  • integrate standardized, formal screening tools and anticipatory guidance into the practice workflow;
  • incorporate innovative approaches and tools for assessing health care quality using family-centered tools; and
  • incorporate techniques for streamlining referral processes and making connections to community support agencies for the provision of developmental family-based services.
