Safety Net Medical Home Initiative: Transforming Primary Care

Studies show that a medical homes are a model of primary care that can reduce costs, while improving quality and eliminating disparities, by placing the patient at the center of care. The Commonwealth Fund is sponsoring a five-year demonstration program to help 65 low-income or safety net clinics in five states transform into patient-centered medical homes.
