In phase 3 of this project, the Manatt/THI team will continue to support the Centers for Medicare and Medicaid Services (CMS) in launching or implementing six to seven of the 45 policy changes outlined in the Investing in Health action plan in the next year. The activities will center around three areas — payment, measurement, and infrastructure — and leverage four strategies: 1) mapping the pathway to change; 2) aligning stakeholders with a vested interest in those changes; 3) partnering with states; and 4) providing federal policy guidance and support. One example of policy change is the incorporation of measures of social determinants of health — such as the percentage of beneficiaries screened for food insecurity — into the CMS core measure set for the first time.
