Transforming Care

This project will support five Transforming Care newsletters over the course of 13.5 months. Each will include a feature article of at least 2,500 words; a case study of the same length or an interview with an expert on the topic; infographics; and a roundup of important journal articles identified by the editorial advisory board. Transforming Care will also continue to serve as a research tool for Fund program staff, furnishing information on timely issues and emerging
models of care such as population segmentation, integration of health care and social services, and viable payment models for rural hospitals. The topics for each issue are developed in collaboration with the Health Care Delivery System Reform program.
