High-Need Patients: In Their Own Words
<p>At a time when federal health funding cuts are on the table, patients who have frequent interactions with the health system can provide a much-needed perspective. A new series of videos gives voice to three individuals who each represent a distinct group of high-need patients: people with at least three major chronic conditions, adults under 65 living with a disability, and frail elders with functional limitations. </p><p>As The Commonwealth Fund’s Tanya Shah and Meredith Brown and Healthwise’s Becky Reed explain on <em>To the Point, </em>"understanding their problems with the health system as well as their daily challenges provides insights into what aspects of care are working for people, and what needs repair."</p>
