New Talking Health Episode: Patient-Centered Medical Homes

<p>The evolution of patient-centered medical homes as a cost-efficient team approach to health care is the subject of the new episode of <em>Talking Health</em>, CUNY-TV's Commonwealth Fund–supported series dedicated to health care issues and innovation. In "Patient-Centered Medical Homes," now available on <a href="/publications/video/2012/aug/talking-health-patient-centered-medical-homes">commonwealthfund.org,</a> host Mike Gilliam discusses the success of medical homes—primary care practices that offer ready access to comprehensive, coordinated care—with Commonwealth Fund vice president Melinda K. Abrams, head of the Patient-Centered Coordinated Care Program, and A. John Blair III, M.D., president of Taconic IPA, a 4,000-member physician group in New York's Hudson Valley.</p><p>The show premiered yesterday on CUNY-TV, the City University of New York television station, and will be rebroadcast this Saturday, August 4, at 5:30 p.m. </p>
