<p>After last spring’s deadline for health insurers to submit their proposed rate requests for 2016, there were plenty of eye-catching headlines regarding double-digit increases.</p><p>But as Georgetown University’s Sean Miskell and David Cusano explain in a new blog post, rate requests vary considerably by insurance carrier and location.
