Commonwealth Fund–Supported Work in the Literature

Recent research supported by The Commonwealth Fund and published in the professional literature explored:

  • The INTERACT (Interventions to Reduce Acute Care Transfers) program, which aims to build the capacity of nurse aides and other direct-care staff at long-term care facilities to identify, evaluate, and manage changes in residents' medical conditions—helping to avoid unnecessary hospital transfers (Journal of the American Medical Directors Association, March 2014).
  • The adoption of various structural capabilities—including patient registries, referral tracking systems, and electronic prescribing—that are key to building patient-centered medical homes. Researchers used accreditation survey data to assess adoption levels among small and medium-sized practices involved in medical home pilots in Colorado, Ohio, and Rhode Island (American Journal of Managed Care, July 2014).
