With support from The Commonwealth Fund, the Visiting Nurse Service of New York (VNSNY) and the Center for Health Care Strategies convened seven health plans in a peer-to-peer learning collaborative to explore strategies for providing integrated care for high-need, high-cost patients, including those dually eligible for Medicare and Medicaid, known as "dual eligibles." A new issue brief by Penny Feldman of VNSNY published by the Center for Health Care Strategies provides a framework outlining the elements of high-performance care for this population. It is intended as a guide for states and health plans working to meet the needs of patients with complex needs.
