Australian Minister of Health Susan Ley announced a plan to establish “healthcare homes” for patients with multiple chronic conditions. As part of the $21 million, two-year trial, 65,000 Australians will receive a tailored package of health services to be provided through up to 200 medical practices. The trial period began July 1.
