Coordination Within Medical Neighborhoods

A recent Commonwealth Fund–supported study examined how patient-centered medical homes exchange information and coordinate care with the hospitals, specialists, and other primary care providers that make up their “medical neighborhoods” (Health Care Management Review, April/June 2016). The researchers, led by Shehnaz Alidina of Brigham and Women’s Hospital and Harvard’s T. H. Chan School of Public Health, focused on 13 medical homes in Colorado.
