Tools and Strategies to Sustainably Address Patients’ Social Needs

A growing number of health care providers and insurance plans are taking steps to address their clients’ social needs — even building affordable housing, in some cases. In a new To the Point post, the Commonwealth Fund’s Corinne Lewis and Martha Hostetter say the next challenge is figuring out which interventions work best, how to pay for them, and how to take them to scale.

The Commonwealth Fund has supported several efforts to help the health sector find sustainable approaches to meeting patients’ nonmedical needs. These include:

  • An ROI calculator for estimating the return from mutually beneficial financial arrangements between health care organizations and their community partners
